When the foster cat needs spay/neuter, they need to be dropped off at a Volunteer's house near Petsmart (14 Mile and Haggerty) on a Wednesday at 6:30 AM, and picked up at the same location on Thursday between 7:00 and 8:00 AM. Are you able to do this?* Choose one: Yes No

I am aware that by signing this application, I agree to provide what is needed for the health and safety of any cat fostered in my care for The Love of Cats Rescue and Adoption. I further agree to assume all responsibility for the foster animal(s) in my care while on or off premises. I also agree to assume liability for the animal(s) while I foster for LOCRA. I recognize that in handling cats/kittens, there exists a risk of injury including physical harm caused by the cats/kittens. On behalf of myself, my heirs, personal representatives and executors, I hereby release, discharge, indemnify and hold harmless LOCRA, and its agents, from any and all claims, causes of action or demands of any nature or cause, including medical costs and attorney's fees, incurred or sustained by myself, my family members or visitors from seeking legal action in any way connected with LOCRA, including, but not limited to, cat bites, scratches, accidents or injuries. PetSmart Store has agreed to let LOCRA temporarily place their cats in the store for adoption. This affiliation waives any and all claims against PetSmart and PetSmart Charities. I will provide care for my fosters according to the guidelines provided by LOCRA and agree the foster cats in my home are not my pets. LOCRA retains ownership of all foster cats and they must be surrendered if requested.
